Requirements

 

If you attained your MPT or MSPT from a CAPTE accredited institute:

  • Official transcript(s)
  • Copy of Physical Therapy license

If you attained your MPT or MSPT from a non-CAPTE (or foreign) institute:

  • Official transcript(s)
  • Copy of Physical Therapy license
  • Academic equivalency documentation from member institutions of National Association of Credential Evaluation Services (NACES) or Association of International Credential Evaluators (AICE)
